Cleveland City Schools has partnered with the health department to offer flu shots to students. This year, they will not be accepting paper forms. Instead, parents/guardians will complete the form online. This information goes directly to the health department. Please call the health department if you have any questions at 423-728-7020. Please complete the form before October 15. CCCES students will receive their flu shots on Thursday, October 17.
