There are numerous forms that are used on a regular basis within the system. Please find below a list of forms that you may use appropriately.
In compliance with School Board Policy 2.404, School Support Organizations, only groups or organizations that have entered into a “cooperative agreement” with the schools may use the name, mascot, or logo of a school or the school district to solicit or raise money, materials, property, securities, services or other things of value.
An annual information form is required for each organization by August 1 each year.
